BMI < 18.5

Underweight

BMI ≥ 35 + comorbidity

Evaluate bariatric surgery referral

Discuss referral options when obesity-related conditions are present and the patient is interested.

BMI ≥ 40

Strong bariatric surgery candidate

Review bariatric referral, medical weight-loss options, safety, labs, and long-term follow-up planning.

Medication decision review

TeleDNPnow may discuss GLP-1 medications, topiramate therapy, or bupropion plus naltrexone when clinically appropriate. Medication selection depends on medical history, contraindications, labs, side-effect risk, access, cost, and patient goals.

Lifestyle plan
